I hereby give my consent to care for any injuries sustained while attending Team Soccer Direct Camp. I understand this includes transportation and treatment at a local hospital. I will allow photos/videos of my son or daughter to be taken at Soccer Direct to be used as promotional material. I further hereby release, absolve, indemnify and agree to hold harmless Team Soccer Direct Schools, California Lutheran University, Team Soccer Directs' Coaches and Staff, except and only to the extent of an in the amount covered by any accident of liability insurance. In consideration of your permitting my child to participate in the camp and the activities, and in consideration of the time and effort devoted by the coaches, supervisors and staff of the camp, I assume all risks and hazards incidental of the activities and transportation to and from the activities.
